What Is the GAPP Waiver?
The GAPP program is operated through Medicaid. It was developed to accommodate medically fragile children under the age of 21. CKCG Health Care Services offers skilled nursing and home care for medically fragile children with special needs. Families have flexibility in selecting the medically necessary services to meet the needs of their child.
Goal of the GAPP Program
The Georgia Pediatric Program delivers skilled nursing care in Atlanta, GA to medically fragile children. Personalized care provided in the privacy of the child’s home is administered by skilled nursing professionals and home health aides. Recipients of the GAPP program get assistance with daily living activities and specialized medical support. Wound care, meal preparation, and health monitoring are examples of the skilled nursing care we offer.
Who Qualifies for the GAPP Program?
The GAPP program requires that children be medically fragile and diagnosed accordingly. All participants of the program must be under 21. Care administered by qualified professionals address the fragile needs of children who are eligible for admission to a hospital or long-term skilled nursing care facility. The Alliant Georgia Medical Care Foundation’s (GMCF) Medical Review Team conducts the review and makes the determination.
