First, you need to reach out to your Case Manager. Your case manager will be able to handle the switch for you. Ideally, you should have an agency in mind. When you approach your Case Manager, you simply give her the name of the new agency. When you give them the new agency information, they will submit the pertinent information on your behalf. From there, you wait to be notified by the Case Manager that you have been switched over. The process typically takes two to four weeks to complete in most cases.
